To be considered for admission, please be sure your application packet includes all of the following in order. Place an "X" in the [ ] at the left of each item submitted.

[X] This page as a cover sheet.
[ ] Application for Admission to Graduate School form, typed or neatly printed, and signed at bottom. You may download and print the application or apply on-line here.
[ ] Signature page for Online Applications (Recieved after completing the online application process.)
[ ] Application Fee or Confirmation of Online Payment (Online application signature page is not a receipt of payment. If paying online, you will receive a separate confirmation of your payment.):
  • $30 for U.S. citizens and permanent residents
  • $55 for international applicants, payable by money order or bank draft made payable to KSU Graduate School
[ ] Signed Affidavit of Financial Support. Students will not jeopardize their chances for receiving financial aid by submitting this form. (Internation applicants only)
[ ] Information Form for Application for Graduate Study. (pdf)
[ ] Copy of Graduate Record Exam (GRE) scores. Required for Master's and Ph.D. students; recommended for MSE students if Bachelor's Graduate Point Average (GPA) is under 3.0.
[ ] Copy of Test Of English as a Foreign Language (TOEFL) score. Required for international applicants only.
[ ] Description of work experience. Typed, maximum of one page.
[ ] Statement of academic goals. Typed, maximum of one page.
[ ] Transcript(s) of Bachelor's and Master's degrees. Transcript must show the date and degree. If you cannot enclose an official copy, please enclose a photocopy and have an official transcript mailed to the Department.
[ ] Three reference letters.

December 1: 		Applications for Fall Semester.

Applications for Spring Semester.

Applications for Summer Semester.
